Due to the nature of how we schedule our exams we prefer you complete the Request an Appointment Form or phone us to secure your appointment. This will allow us to obtain your insurance information so that we may maximize your insurance benefits during your appointment. We respect all of our patients time. If your arrival is later than 15 minutes to your appointment time we may have to reschedule.
